The following is required, once complete, send them to Melissa (via email)

1. Completed Business Resolution Authorization form (Original)
2. EIN number
3. Articles of Incorporation
4. Certified Copy of Corporate Resolutions (Bylaws)
5. Min opening balance of $100.00
6. Utility bill (Mark asked if we can do without)
