  • How did you find out about MediaWiki
This should include something like "Wanted something like what Wikipedia had" or "Wanted my site to be like Wikipedia"
  • How many sites do you manage?
should be multiple choice including 0 as a choice. then ranges like 1, "5 or fewer", "six or more" -- we don't need too much detail.
  • How many end users?
Should tell them to look at "Active users" on Special:Statistics on their wiki
  • What version?
Should tell them to look at Special:Version on their wiki. May want to have multiple choice with latest supported major versions and "not listed".
